" NEW" Proof of Vaccination Requirements
Submitted By SLMH on Tue 12 14, 21
Please note the following Provincial requirements that will be put in place at Shallow Lake Arena over the next couple of weeks:

  • Next Monday (December 20th) we will be checking vaccine records of all arena patrons 12+. Until now, this wasn’t necessary as those between the ages of 12 to 17 years old that were participating in organized sports (specifically provincially recognized organizations including minor hockey and skating club) were exempt. This group is no longer exempt as of the 20th and we will be checking vaccine certificates and having them sign the contact tracing log the same as any other arena user.
  • Starting January 4th, we are required to use the Verify Ontario application to check vaccine records of all eligible arena users. Please make sure everybody 12 years and older enter the arena have an enhance vaccine certificate with QR Code ready to present to staff for scanning. You will not be permitted entry without this certificate, old versions without the QR Code will not be accepted and visual inspection is no longer sufficient. Proof of identification is still required (health card, drivers license, etc.). Information for obtaining your enhanced vaccine certificate can be found here:https://covid-19.ontario.ca/get-proof/

It is recommended for those that use their smartphone to provide proof of vaccination, a backup hardcopy be kept should you have any problems (battery dying, phone damage, etc.)

  • Starting January 10th people with medical exemptions will have to have their physician’s letter reviewed by the Health Unit so they can receive a QR Code for us to scan with the Verify Ontario app. We cannot accept letters at the door after this date.
