First Time Players
Submitted By SLMH on Mon 04 08, 19
SLMH is again offering FREE registration for all FIRST TIME HOCKEY PLAYERS.

This is made possible by Shouldice Designer Stone and their continued support for minor hockey.

If your child(ren) have never played before, and you want to sign, but need step by step help with the process please email Allison at [email protected]

Families are required to provide a scanned copy of their child’s birth certificate, plus mileage from their home to the Shallow Lake Post Office (Allan’s Convenience Store) and their home to the next closest hockey center’s post office.  The difference between these two distances must not exceed eight (8) km. If it does, the player does not have the "Right of Choice" and must play in their closest centre.

If it is less than 8 km, the player has the right to choose which centre to play in and must complete a
[Right of Choice] Declaration which is available on our website under “Registration tab”

If mileage information is not available, registration will NOT be accepted.

Also required is the Respect in Sport Parent Program which is an online proactive, educational program that empowers parents with the tools to ensure the game is enjoyable and respectful for themselves, their children and all other stakeholders in the game.

At least one parent or guardian of each SLMH player registered will be required to have completed the online Respect in Sport Parent Education Program as a condition of their child’s participation.

The cost of the online Respect in Sport Parent Education Program is $12 plus HST which can be paid via credit card or PayPal when registering online.

IMPORTANT:

Upon completion of the program please ensure to save a copy of your certificate and the confirmation email for future reference.

Deadline for registration is Friday, September 6

** Cash calendar fundraising ($150) is in effect for ALL players **
