Required For Tyke and Novice Players

You may have received an email from our Registrar asking for a copy of your child's birth certificate and mileage from your home to the Shallow Lake Post Office, and your home to your next hockey centre's Post Office.  We understand you may have already provided SLMH with this information, however, we have discovered that this information has not been recorded with your child's Hockey Canada profile.     

We apologize for asking again. but it is mandatory in order for a player to be rostered on a team.  Thank you in advance for understanding.
